NDIS Services
Mindful Pathways Support provides structured, practical NDIS supports across Perth metro, delivered at home and in the community. We support children, young people, and adults with a wide range of disabilities and support needs. Our approach is calm, consistent, and focused on everyday outcomes such as routines, community participation, and building skills in real world settings. Support is aligned with individual goals and designed to fit real day to day life.
We accept NDIA managed, plan managed, and self managed participants.
Supports are delivered at home and in the community across Perth metro,
aligned with NDIS goals and everyday routines.
Daily living and personal care
Support with daily routines in a respectful, steady way. This can include prompting, hands-on assistance where required, and support to build confidence with everyday tasks.
